I just noticed it looks like you ran “bokeh serve surface3d/main.py”, is that correct? To run “directory style” apps, you need to start by passing the directory to bokeh server, i.e.

    bokeh serve surface3d

Apart from that, I am not sure. I just tested the app in both python 2 and python 3 environments with bokeh 0.12.2 installed, and they functioned as expected.
